Open presents Version to musical chairs.  You have the children sit in a circle around the birthday child.  Now use a object that matches your theme - such as my son's last birthday party was fish and you would use a fish.  Each guest has the present behind them and you play the music.  Then when you stop whom ever has the fish then hands the present they bought to the birthday child to open.
